Narrator is a small utility that comes in every Windows XP/2000/Vista package. It's main role is to read aloud on-screen text on dialog boxes, menus, and buttons.

In order to use Narrator you have to have a sound card and speakers (or headphones). Windows 2000 was the first that introduced Narrator as a good helper for blind people.

Windows 2000 uses SAPI 4 and allows the use of other SAPI 4 voices.
Windows XP version uses the newer SAPI 5, but it only allows the use only of the default voice of Microsoft Sam.
Windows Vista uses SAPI 5.3 version and allows usage of Microsoft Anna voice or the Microsoft Lili voice (for Chinese systems).
